本實驗室對心血管力學的研究如下:
(1)發展指數函數之非均勻性T管模型(exponentially tapered T-tube model)以探討體循環動脈管之物理性質。吾人發現老鼠之血管阻力與特徵阻抗隨年齡的增加而下降,然而量化大動脈管硬度(arterial stiffness)的脈波傳輸時間卻不受年齡所影響,這意謂著直至十八個月大的老鼠,其大動脈管以及阻力性血管皆有擴展(dilatation)的現象。吾人亦發現節食可降低血管阻力以及特徵阻抗,然而卻不影響脈波的傳輸時間,這意謂著經過半年的隔天給食,支配十二個月大老鼠之心血管的交感神經興奮性有降低的現象。雖然節食可增加十二個月大及十八個月大老鼠的動脈容積度(arterial compliance),但其機制卻不盡相同。Nichols教授(血行力學經典之作"McDonald's Blood Flow in Arteries" 1990和1998年版作者之一)曾撰寫一篇具肯定性的文章發表於2000年一月份的Journal of Gerontology-Biological Sciences,此評論對本實驗室之鼓勵甚巨。
(2)運用彈性-阻力模型(elastance-resistance model)來探討血管之收縮力學(systolic mechanics)。吾人發現老化老鼠心室收縮力的下降可能肇因於調控心肌長度的鈣離子敏感性的機制(length-dependent mechanism for Ca2+ sensitivity)受到傷害,此傷害可經由節食所導正而恢復心室收縮力;此外,老化鼠等效動脈彈性的降低可能導致心室內部收縮阻力的下降,然而心室內部阻力之下降不受節食所影響。吾人亦發現,以streptozotocin (STZ)所誘發之早期糖尿病鼠具有與老化鼠相同趨勢之心臟收縮力學的變化。
(3)使用彈性模型(time-varying elastance model)來探討心室-動脈管之交互作用(ventricular-arterial coupling or interaction)。當心室之末期收縮彈性與動脈之等效彈性相匹配時,動脈可從心室獲取最大的能量。吾人發現STZ-糖尿病鼠之等效動脈彈性的下降遠超過心室末期收縮彈性的下降,這將降低左心室傳輸至動脈管的能量。本實驗室亦評估liriodenine與quinidine之類抗心律不整的藥物對心血管交互作用的影響,發現在有效劑量下,liriodenine幾乎不影響心臟與血管的力學以及心室-動脈管的交互作用。反觀quinidine則相當程度地減少等效動脈彈性而降低心室與動脈管的匹配,這使得能量從左心室傳輸到動脈系統受到傷害。
(4)吾人使用核醫心臟攝影所得之時間-活性曲線(time-activity curve)來探討心室之舒張功能。基本上,等體積舒張期、快速填充期、diastasis以及心房收縮期很難界定。本實驗室發展一套電腦計算規則可自動界定二尖瓣的開啟(mitral valve opening)、快速填充期的結束以及心房收縮的開始。吾人發現,在尚無心室肥厚的情況下,高血壓病患因其大動脈管的硬化使得脈波反射增強而延長了等體積舒張期,這可為心室收縮功能趨壞的先期指標。未來,在臨床上期望能近一步探討年齡與性別對心室之舒張與收縮功能的影響,以及糖尿病對心室之舒張與收縮功能的影響。

Kuo-Chu Chang was born in Taiwan in 1952. In 1991, he received his PhD degree from the Section of Biomedical Engineering, Institute of Electrical Engineering, National Taiwan University. He is now Professor in Physiology at the College of Medicine, National Taiwan University. His main research interests include the modeling of the systemic arterial tree and the systolic mechanical properties of the ventricular pump.
